Canossa Private Hospital has built a reputation of excellence in providing inpatient and outpatient rehabilitation programs over the past decade.

We offer rehabilitation for:

Orthopaedic conditions e.g. following fractures, joint replacement surgery, musculoskeletal conditions, people with a history of falls etc.

•

Neurological conditions e.g. Stroke, Parkinson’s Disease, Motor Neurone Disease and Movement Disorders.

•

General re-conditioning to improve mobility and functional independence after recent illness, pre or post surgery, chronic diseases causing debility and loss of function etc.

Accommodation

The Rehabilitation Unit has 36 inpatient beds and several large day therapy areas. It is a modern facility in which patients have their own private room. There are spacious gymnasiums, dining rooms and common areas. Onsite parking is available free of charge to visitors.

On Admission
On admission you will be welcomed and orientated to the unit by a nurse. The first days of your stay will involve numerous assessments by your admitting doctor, nurses, physiotherapists, occupational therapists and other allied health staff as required. An individualized rehabilitation program will then be developed for you using the results of these assessments. As you progress through your program you will be involved in your care decision making and planning for discharge.
Program
All rehabilitation programs are managed by a multidisciplinary team of experienced health professionals which includes:
•

Rehabilitation Physicians/Geriatrician – visit regularly to manage medical issues and monitor your progress.

•

Physiotherapy to gain maximum independence in mobility, balance and physical function.

•

Occupational Therapy to improve independence and safety in daily occupations such as self care and domestic and community activities.

•

Speech Pathology to re-train speech and language as well as provide advice on dietary changes e.g. if swallowing problems are evident.

• Rehabilitation Nurses to support and reinforce the goals of the program.
• Dietitian to educate in dietary modification where required.
• Social Workers to provide comprehensive practical discharge assistance and advice for discharge and beyond.
Therapy sessions are held in the morning and afternoon depending on individual patient needs and ability. Participation by the patient is essential to achieve their rehabilitation goals.
Discharge Planning

The Rehabilitation Team meets weekly to monitor and review your progress.


A home visit may be conducted to enable safe access and independence in the home. This may include minor modifications to your home and the purchase of specialised equipment. A meeting will be held between the patient, family members (where relevant) and the Rehabilitation Team to discuss future plans.

Discharge will be organised when the rehabilitation goals have been met. A referral to community services will be made when required e.g. for ongoing therapy, domestic services, community nursing services, meals assistance etc.

Outpatient Rehabilitation
Arrival at the designated time is important so that your admission can be processed quickly. It is recommended that you park in the car park out the front of the building for ease of access. You will be required to attend your scheduled therapies on scheduled days during the week.  You will need to wear comfortable clothes with well fitting  and comfortable closed in shoes with low heels.
